Participant Approval Forms All trip reservations are subject to approval by the trip leader. After you register for a trip, we mail you a reservation confirmation packet, whether you're on the trip or waitlisted. Each packet includes three forms that all applicants must complete: Individual liability waiver (more information) Medical form Participant approval questionnaires (listed below) Your leader may also ask you to fill out a travel information form. You may be canceled from a trip if you don't submit these three forms to the leader within 30 days of making your reservation. If you are completing forms that you received in your confirmation packet, then you will find your leader's address printed on the forms. If you need to download your forms here to expedite the approval process, you will need to contact us for the leader's address information. The leader reviews these forms and makes sure that each trip applicant has the appropriate equipment, experience, and physical conditioning to participate in the outing. The leader will contact you if you are not approved for the trip. In this case, we will refund any reservation payments you have already made, or we can transfer them to another, more suitable trip.
